Information given in the\u00a0questionnaire will be used to establish if a Wastewater Discharge Permit is required.<\/p>\n<p>Please fill out and submit the wastewater questionnaire below.<\/p>\n<p>If there are any changes in the information contained in the\u00a0original submitted questionnaire or changes in the numerical values outside of the ranges stated in the\u00a0questionnaire,   the user is required to submit the changes,   in writing,   within 30 days of occurrence.
